AFO Pads

AFO pads (ankle-foot orthosis pads) are soft, cushioned inserts designed to sit inside an AFO brace, improving comfort and helping protect the skin where the device contacts the foot, ankle and lower leg. 

They’re a simple upgrade that can make a big difference for day-to-day wear - especially when an AFO is being used for gait support and alignment in conditions such as cerebral palsy, stroke, or spinal cord injury. 

Our AFO pad range focuses on pressure relief and wearability. Using foam and other cushioned materials, these pads help reduce rubbing and shear, add targeted padding where it’s needed most and support better tolerance of longer wear times without changing the core function of the orthosis itself.

Selection Guide: Using AFO Pads Effectively

AFO pads work best as a “micro-fit” tool: identify the exact hotspot, add cushioning only where needed and confirm you’ve improved comfort without compromising heel seat, suspension, trimlines or alignment.

1) When to Use AFO Pads (and When to Be Cautious)

Good use-cases

  • Localised rubbing at common contact points (e.g. malleoli, navicular, instep, tibial crest, fibular head, Achilles/heel cup edge)
  • Mild volume fluctuations through the day (common in neuro/vascular patients)
  • Sensitive skin, scar tissue, reduced sensation or paediatric tolerance/compliance challenges

Be cautious / avoid

  • Open wounds, rapid skin breakdown, suspected infection (pads are not wound dressings)
  • If added padding lifts the heel, reduces heel seat depth, or causes the foot to “float”
  • If bulk compromises footwear fit and forces upsizing (often creates new pressure points elsewhere)

2) A Simple Step-by-Step Workflow

  1. Map the problem properly
    • Ask the patient to indicate the painful area and confirm with redness patterns after wear.
    • Mark the AFO (internal contact area) and the limb (skin location) to match hotspot to shell/liner.
  2. Choose the padding strategy
    • Friction/rubbing: use a thinner, low-profile pad to reduce shear and edge irritation.
    • Pressure point: use slightly thicker, localised padding to spread load and offload the prominence.
    • Trimline/edge irritation: address the edge first where possible; pad can help, but don’t “bury” a sharp trimline under bulk.
  3. Start small
    • Cut a trial piece that is slightly undersized. You can increase coverage later.
    • Always taper/feather the edges to avoid creating a new ridge (the next hotspot).
  4. Prep the surface
    • Clean and dry the pad application area (oils, talc, creams reduce adhesion and increase pad migration).
    • Dry-fit position before fixing permanently (especially around malleoli and trimlines).
  5. Apply and seat
    • Apply the pad, then re-check position after donning—pads can shift when the limb is inserted.
    • Confirm the pad isn’t pushing the foot medially/laterally or changing heel position.
  6. Functional check
    • Reassess heel seat, strap tension, swing clearance, and stance stability.
    • Do a short walk test and re-check for immediate edge roll or migration.
  7. Patient handover
    • Advise: if a new pinch/hotspot appears, stop wear and report - don’t “wear through it.”
    • Explain pads compress over time and may need replacement or repositioning.

3) Cutting & Placement Tips (That Prevent Callbacks)

  • Feather edges: bevel or thin the perimeter to prevent a hard step that creates friction.
  • Keep it local: local issues usually want local padding—avoid full-length padding unless intentionally adjusting volume.
  • Don’t mask a trimline issue: if the shell edge is the culprit, adjust/finish the trimline where appropriate before adding bulk.
  • Watch alignment creep: thick pads at the malleoli can subtly shift calcaneal position and alter control.

4) Common Hotspots & Practical Fixes

  • Medial malleolus: use a small offload (“donut” style) approach where possible - avoid a large slab that shifts the foot.
  • Lateral malleolus: same principle - minimal, tapered offload to reduce varus/valgus influence.
  • Tibial crest: a thin, slightly wider strip spreads load; taper edges carefully.
  • Heel cup/Achilles rim: pad the edge area if rubbing, but preserve heel seat depth and avoid lifting the heel.

5) Quick Troubleshooting

  • Pain moved: pad is too big/thick or poorly feathered - reduce thickness, taper more or reposition.
  • AFO feels loose: padding may be in the wrong zone - adjust padding nearer suspension/containment areas (without over-bulking).
  • New redness line: pad edge is acting like a new trimline - re-cut with a longer taper/feather.
